- Die Studie untersucht, warum sich Prädiabetes bei manchen Menschen innerhalb eines Jahres zu Diabetes entwickelt, während andere wieder normale Blutzuckerwerte erreichen. Dazu wurden die Konzentrationen von 1.389 Proteinen im Blut von Personen mit Prädiabetes, neu diagnostiziertem Diabetes oder normaler Glukosetoleranz analysiert. Bei Personen mit Prädiabetes, die innerhalb eines Jahres Diabetes entwickelten, waren die Konzentrationen von 14 Proteinen im Blut signifikant erhöht im Vergleich zu Personen, die wieder eine normale Glukosetoleranz erreichten. Diese Proteine standen in engem Zusammenhang mit den Blutzuckerwerten und korrelierten stärker mit der Insulinsensitivität als mit der Betazellfunktion. Besonders die Proteine DCXR und GSTA3 waren bereits im Prädiabetes-Stadium mit einem erhöhten Risiko für die spätere Diabetesentwicklung verbunden. Zudem zeigte die Analyse, dass Veränderungen in immunologischen Signalwegen bereits vor dem Ausbruch des Diabetes vorhanden waren, was auf eine wichtige Rolle von Entzündungsprozessen bei der Krankheitsentstehung hinweist. Die Ergebnisse deuten darauf hin, dass nicht nur eine gestörte Betazellfunktion, sondern insbesondere eine verminderte Insulinsensitivität und Veränderungen des Immunsystems zur Entwicklung von Diabetes beitragen. Die Proteine DCXR und GSTA3 könnten künftig zur Früherkennung von Personen mit hohem Diabetesrisiko dienen. Zudem stärken die Befunde die Annahme, dass chronische unterschwellige Entzündungen eine wichtige Rolle bei der Entstehung von Diabetes spielen. Dies könnte neue Ansätze für Prävention und Therapie eröffnen, die auf die Verbesserung der Insulinsensitivität und die Regulation des Immunsystems abzielen.
- Abstract (EN)
- The study investigates why prediabetes progresses to diabetes within a year in some people, whilst others return to normal blood glucose levels. To this end, the concentrations of 1,389 proteins in the blood of individuals with prediabetes, newly diagnosed diabetes or normal glucose tolerance were analysed. In people with prediabetes who developed diabetes within a year, the concentrations of 14 proteins in the blood were significantly higher compared with those who returned to normal glucose tolerance. These proteins were closely linked to blood glucose levels and correlated more strongly with insulin sensitivity than with beta-cell function. In particular, the proteins DCXR and GSTA3 were already associated with an increased risk of subsequently developing diabetes at the prediabetes stage. Furthermore, the analysis showed that changes in immunological signalling pathways were already present before the onset of diabetes, suggesting that inflammatory processes play an important role in the development of the disease. The findings suggest that it is not only impaired beta-cell function, but in particular reduced insulin sensitivity and changes to the immune system that contribute to the development of diabetes. The proteins DCXR and GSTA3 could in future be used for the early detection of individuals at high risk of diabetes. Furthermore, the findings support the hypothesis that chronic low-grade inflammation plays an important role in the development of diabetes. This could open up new approaches to prevention and treatment aimed at improving insulin sensitivity and regulating the immune system.
