- Die online-adaptive Protonentherapie (OAPT) ist ein vielversprechender Ansatz, um gesundes Gewebe von Patienten effektiver zu schonen, indem Behandlungsunsicherheiten der Protonentherapie minimiert werden. Hierbei könnten in-vivo-Reichweiteverifikationsmethoden als unabhängiges Qualitätskontrollinstrument zur Triggerung und Überprüfung von Behandlungsanpassungen eine zentrale Rolle spielen. Als Voraussetzung hierfür wurde in dieser Arbeit die automatische Auswertung von Prompt-Gamma-Imaging (PGI) Daten mithilfe von künstlicher Intelligenz (AI) untersucht. Zu diesem Zweck wurden AI-Modelle unterschiedlicher Komplexität auf Basis realistisch simulierter PGI-Daten entwickelt, um Behandlungsabweichungen zu detektieren und zu klassifizieren, mit dem Ziel die Modelle auf reale Patientendaten eines klinisch genutzten PGI-Detektorsystems zu überführen. Für simulierte PGI-Daten zweier Tumorentitäten wurden exzellente Ergebnisse erreicht. Bei realen klinischen PGI-Daten erreichte das beste Modell eine Testgenauigkeit von 0.75, was das Potential von PGI als Verifikationssystem in einem OAPT-Workflow verdeutlicht. In einer ersten Machbarkeitsstudie konnte zudem gezeigt werden, dass die PGI-Daten der ersten Behandlungsschichten von Prostatakrebs-Patienten bereits genügend Informationen enthalten, um den Großteil der Behandlungsabweichungen zu detektieren.
- Abstract (EN)
- Online adaptive proton therapy (OAPT) is a promising approach to spare healthy tissue of patients more effectively by minimizing treatment uncertainties in proton therapy. In such adaptive workflows, in-vivo range verification methods might play an integral part by acting as an independent quality control tool to trigger and verify treatment adaptations. As a prerequisite, this thesis investigated the automatic evaluation of prompt gamma imaging (PGI) range verification data using artificial intelligence (AI). For this purpose, AI-based approaches of varying complexity were developed to detect and classify treatment deviations based on realistically simulated PGI data and then translated to real-world patient data from a clinically used PGI detector system. Excellent performance was reached for simulated PGI data of two tumor entities. For real-world clinical PGI data, the best performing model achieved a test accuracy of 0.75 indicating that the current PGI system could already be used as a verification system in an OAPT workflow. Additionally, a proof of concept investigation suggested that PGI data of the first few treatment layers of prostate cancer patients already contained sufficient information to detect the majority of treatment deviations.
